ADMISSION

Request for Admission 2017-2018 Academic Year

Admission to Excelsior Charter School of Broward is limited to grades k through 5.   Scores on the entrance examination, school records (and in certain situations an interview) are all used to determine admissions.

Your admission application is available here for you to print, fill out and submit to the school. Please complete each area and return via mail to: 10066 W. McNab Road, Tamarac, FL. 33321.  Only Original forms are accepted, please no fax copies.

Registration include:  Student Registration Packet 2017-2018 Excelsior Charter

  • Student Registration Form
  • Student Records Request (Please sign and date where requested.  This form must be completed in order for Excelsior Charter to request records if you are a transferred student)
  • Emergency Information Card
  • Authorization for Medication (Use only if student is taking medication upon entry to school, not all medication deliveries may be facilitated in school)
  • Parent Contract
  • Student Contract
  • Technology Acceptable Use Policy
  • Photo and Video Release
  • Home Language Survey (must be completed by all families)
  • Student Disclosure Form
  • Special Education Program Information (If your child currently has an IEP or Section 504 Plan, you must meet with the school’s ESE Specialist to determine program needs prior to acceptance.  In the event the IEP or Section 504 Plan is not disclosed, the student’s status in the school may be jeopardize.

Items needed for registration if transferring from Broward County Public School

  • School Registration Form
  • Copy of the most recent report card

Items needed if transferring  from Private, Charter, or Out of County School

  • School Registration form
  • Record Request
  • Copy of Birth Certificate
  • Original (Blue) Health Immunization Form 680
  • Original (Gold) Physical Form
  • Copy of the most recent Report Card
  • Proof of address (Must have two  forms; i.e. deed, lease, utility bill etc.)
